Purpose of the processing of personal data
In accordance with Article 6 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (hereinafter referred to as the General Data Protection Regulation), the data provided in this Monitoring Sheet of the supported person are collected and processed solely for the purpose of demonstrating the proper and efficient use of the funds provided from the Asylum, Migration and Integration Fund Operational Programme (AMIF OP) for the implementation of the project. The data from this form will be used for the preparation of reports on the implementation of the project. This form may be inspected by the staff of the implementing structure of the programmes concerned or by persons authorised to carry out the inspection, provided that confidentiality of all the data inspected is maintained.

Legal basis for the processing of personal data
The Ministry of the Interior, as the controller of personal data processed in connection with the implementation of projects supported by the OP AMIF, is authorised to process the personal data of the supported person mentioned in this form on the basis of Regulation (EU) No 2021/1060 of the European Parliament and of the Council of 24 June 2021 on common provisions for the European Regional Development Fund, the European Social Fund plus, the Cohesion Fund, the Equitable Transition Fund and the European Maritime, Fisheries and Aquaculture Fund and on the financial rules for these funds and for the Asylum, Migration and Integration Fund, the Internal Security Fund and the financial support for border management and visa policy instrument. The implementer of a project supported by the OP AMIF is entitled to process the personal data of the supported person mentioned in this form on the basis of a mandate issued by the controller (i.e., the Ministry of the Interior) in accordance with Article 28 of the General Data Protection Regulation.
